Team evaluation form

Click here for the team evaluation form for the term project. This form is to be turned in during the practicum, Saturday July 1, at 1:00. There should be one form filled out for each person, not one form per team. When you fill out your form, put your team name on the top, and list all members of the team, including yourself. Write a percentage next to each person's name indicating the amount of contribution that person made to the project. Your score will be the average of all forms turned in for your team, multiplied by the number of forms turned in for your team. I will bring these forms with me to the practicum.
